Guided Discovery CBT Worksheet

Thoughts in our brains are very fast and unfiltered. Sometimes we don’t get time to analyze and
challenge them. Spend some moments to analyze your thoughts and answer the questions
given below:

  • Mention your thought
  • Rate its intensity on a scale of 1-10
  • What is the evidence in favor of this thought
  • What is the evidence against this thought
  • Do you think your thought is based on true scenarios or you are exaggerating the
  • consequences
  • What could be the worst outcome
  • What could be the ideal outcome
  • What could be the realistic outcome
  • If your friend had the same situation or thought what would you suggest to them
  • Now what is your suggestion to yourself
  • Rate the intensity of the above-mentioned thought now on a scale of 1-10
